Patient Forms
Upon entering our office, our receptionist will welcome you as a member of our family. Your new patient paperwork will have been previously FULLY completed on our website and downloaded to your secure patient file, so additional paperwork should be minimal. This paperwork provides us with your health history and information on your condition, so the more information you give us, the more comprehensive and beneficial your examination will be.
Note: PLEASE BE AS DETAILED AS POSSIBLE IN STATING THE CHRONOLOGICAL HISTORY OF YOUR COMPLAINTS.

Payment Options
Southwest Brain Performance Centers believes in the value of the right diagnosis and right treatment in the shortest amount of time. Our office does not participate in any private health insurance programs. We do accept auto accident cases that have MEDPAY coverage and workers compensation cases. Also, Flexible Spending Accounts cover chiropractic care. To learn more about our payment options for chiropractic and wellness care, you can call our office directly.
Insurance
Why do we not accept health insurance? Unfortunately, the amount that the insurance programs reimburse for chiropractic care is very poor. Dr. Pendleton believes that he can give more time and attention to cash paying patients instead of having to see many more patients per hour to make up for the amount he would be losing from private health insurance reimbursement. You will treated for your specific diagnosis with the goal being to get you back to health as soon as possible! Should you desire, our office will give you a receipt with diagnosis and charges that you can use to file with your insurance company to be considered for reimbursement to you.
At Southwest Brain Performance Centers we're convinced that a small amount of your time and money spent protecting your most important asset--your health--will be far less time consuming and expensive than trying to recover it. So please, we would like to encourage you to take your health very seriously. As the old saying goes, if you don't have your health, you don't have anything. Unfortunately, too many people don't discover how true that statement is until it is too late.
